Most of the children we serve are covered through the Georgia Pediatric Program. We help families and case managers navigate GAPP — and CCSP and SOURCE for adults — so care can begin quickly.

Georgia Pediatric Program

FINS Home Care proudly provides services through Georgia's Pediatric Program (GAPP), helping medically fragile children receive skilled nursing care in the comfort of their homes.

We care for infants, toddlers, children, and teenagers with complex medical conditions who need continuous nursing support.

Our nurses provide compassionate care for children who may use ventilators, feeding tubes, tracheostomies, oxygen therapy, specialized medical equipment, or require ongoing assessments and skilled nursing interventions.

Every child deserves the opportunity to grow, play, learn, attend school when possible, spend time with family, and experience life in a safe home environment.

We work alongside physicians, hospitals, therapists, schools, and families to create individualized plans of care that support each child's medical needs and developmental goals.

GAPP Eligibility

How families qualify for pediatric nursing hours

GAPP serves Medicaid-eligible children under 21 who are medically fragile and need physician-ordered skilled nursing or personal care support at home.

We handle the paperwork with you — at no cost — and coordinate directly with your child's physician and case manager.

  • Child is under 21 years old and Medicaid eligible
  • A physician orders medically necessary skilled nursing or personal care support
  • Documented medically complex or technology-dependent needs
  • Nursing assessment and plan of care
  • Prior authorization submitted for nursing hours
  • Ongoing reviews as your child's needs change

CCSP & SOURCE

Adult Medicaid waiver programs

Alongside our pediatric focus, we serve adults through the Community Care Services Program (CCSP) and SOURCE (Service Options Using Resources in a Community Environment) — Georgia Medicaid programs for people who meet nursing-home level of care but prefer to remain at home.

FINS Home Care provides supportive services that help adults maintain independence and receive assistance with daily needs. Services may include:

  • Personal care assistance
  • Daily living support
  • Meal preparation
  • Companionship
  • Safety monitoring
  • Household assistance
